Centered around a telemedicine model, but with in-person appointments also available, expanding the reach of mental health services to those who might not be able to regularly travel to office visits is important.

From Anxiety & Depression, to ADHD, PTSD, LGBTQIA+, and even Long COVID, the providers at CCPW are contemporary with their training and provide therapies designed to maximize every patient's quality of life.

​

Business and medical minds came together to provide a much needed service to patients in communities across Connecticut.  A revolutionary mental health and well-being clinic was launched in late 2023.

BECOMING A PATIENT

We welcome all who chose to become a patient of our practice, whether for one visit, or a specifically designed course of care.  For everyone's comfort and well-being, we ask cooperation, respect, and adherence to our prescribing policies of all who choose to benefit from the great providers at CCPW.

The office or your provider might ask your help in completing some forms necessary to begin your treatment.

Simply give us a call or have your primary care doctor send us a referral form.  We'll handle the rest.

​

We gladly accept CT Medicaid (Husky), Medicare, and Commercial Insurances.  Read our payment policy.

​

We take the responsibility of welcoming all our new patients seriously and work hard as a team to ensure patients experience the best treatment and compassionate care.

PLEASE NOTE:  Due to healthcare and medication prescribing regulations, we cannot take you on as a patient and schedule your first visit until all information, insurance, and consents are complete and received by our office.  Our office staff will work with you and help you complete this information in the most convenient way possible.

REFERRING A PATIENT

If you're a healthcare professional, the staff and providers at CCPW will graciously accept your referrals and treat your patients with the dignity, respect, and care they deserve.  Simply give us a call or have your staff fax over a referral form.  We'll take it from there.  We'll reach out to your patient and work to connect them with the right medical provider, at a time that's convenient for them.  We'll even handle insurance authorizations and medication management.

Referring your patients to CCPW is a responsibility we do not take lightly.  We're here for your patients and your office too.

As of January 1, 2022, under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
